Futurity.org ? For cancer, exercise advice beyond 'just do it'

U. ROCHESTER (US) ? Four new studies bring doctors closer to being able to offer patients with cancer specific advice about the benefits of exercise.

Exercise generally helps the nation?s 12 million cancer survivors, but researchers are still working toward being able to prove, with scientific certainty, that prescriptions for daily yoga or 20 minutes of walking will likely extend a patient?s survival.

Understanding specifically how exercise benefits subpopulations of cancer patients is among the big topics at the American Society of Clinical Oncology (ASCO) 2012 annual meeting in Chicago from June 1-5.

?In 15 years we?ve gone from being afraid to recommend exercise to people with cancer, to having enough data that shows, by and large, it is safe and effective, particularly for relief of treatment side effects,? says Karen Mustian, assistant professor of radiation oncology at the University of Rochester Medical Center (URMC) and an exercise psychologist specializing in cancer.

?But when a patient walks in the door, he or she wants to know how to tailor exercise to their own situation. Should I bike, walk, or lift weights? Is there anything I should avoid? Can certain exercises lower my fatigue? And the scientific community has not settled into a place where we?ve developed explicit exercise prescriptions, or can effectively narrow the choices for patients.?

Several studies from URMC researchers suggest that knowing how to develop and apply specific exercise programs (dose and timing to achieve a certain outcome) is a realistic goal.

?Staying Bold While Old?

Mustian and research assistant professor Lisa K. Sprod investigated physical activity among older people, a group they describe as often overlooked and underestimated in terms of their ability to live with cancer. On June 2 at ASCO they will present two studies in a session titled ?Staying Bold While Old.?

Mustian conducted a phase II clinical trial to discover whether six weeks of a home-based exercise plan (walking and resistance bands) improved cancer-related fatigue and strength in 58 men with prostate cancer who were treated with radiation and androgen deprivation therapy. The mean age of the group was 67. The men had a wide range of fitness levels, from frailty to golfers who were fit and active. A control group that did no exercise was also established.

Researchers measured cardiopulmonary function (through a gold-standard test called Vo2-peak) and muscular strength, and found that all exercisers improved while the control group declined in performance. The most significant information, Mustian says, was that everyone who exercised regularly achieved some added benefit?even the fittest participants.

A larger study is planned to explore whether the physiological effects of exercise are responsible for improvements, or if a psychological component is also present, such as the personal attention one often gains from an exercise program, or the camaraderie of being with friends at a gym.

In a second study, Sprod investigated the appropriate amount, type, and intensity of exercise in cancer survivors older than 65, who are also experiencing the natural functional declines associated with aging. The double hit, she says, is an understudied issue.

After analyzing a national sample of 14,887 people, Sprod established that older cancer survivors engage in less physical activity?even routine activity such as stooping, lifting, and walking?than people without a history of cancer. This may lead to less independence, a higher risk of the cancer coming back, and reduced survival.

The study raises new questions, such as whether the treatment contributes to less activity, or whether patients and physicians are worried about the safety of becoming physically active during and after cancer treatment, or a combination of the two factors, Sprod says.

In another exercise-related study, Luke J. Peppone, a research assistant professor, investigated the effects of a yoga program on women with breast cancer who were taking aromatase inhibitors?medications that deplete estrogen and often cause severe menopause-like symptoms. A frequent complaint is joint pain and muscle aches, sometimes making it difficult for women to get out of bed or grip a fork and knife.

Peppone says a high percentage of breast cancer survivors discontinue aromatase inhibitor therapy because of side effects, putting them at greater risk for cancer recurrence. His study measured the self-reported quality of life and physical discomfort among 95 women taking aromatase inhibitors and 72 women who were not taking the drug.

Each group took part in a four-week gentle yoga program. The women taking aromatase inhibitors reported a significant reduction in pain, muscle aches, and total physical discomfort, Peppone says. The study will serve as a pilot to launch a larger study of exercise to relieve musculoskeletal pain.

Peppone and Sprod?s studies have been selected for presentation at the Multinational Association of Supportive Care in Cancer meeting in June in New York.

Finally, a controlled study of yoga therapy in middle-aged, mostly female cancer survivors showed that a four-week, customized gentle yoga plan significantly reduced perceived difficulty with memory.

Michelle C. Janelsins, research assistant professor at URMC?s James P. Wilmot Cancer Center, randomized patients to one of two groups: standard follow-up care after receiving adjuvant cancer treatment, and standard care plus a program that consisted of breathing exercises, gentle Hatha and Restorative yoga postures, and meditation. The yoga group had significantly reduced memory difficulty compared to the standard care control group.

Improved memory also partially contributed to improved fatigue and improved quality of life, Janelsins notes.

In some ways, Mustian says, the exercise-and-cancer dilemma harkens to the days when it was not known what types of chemotherapy and how much should be given to cancer patients. Through rigorous scientific inquiry, physicians were able to refine the way they prescribe and administer chemo?and Mustian believes the same refinement is possible with exercise.

?I think we do an incredible disservice to people who have just been diagnosed with cancer when we say, ?Just do it!??Mustian adds. ?Sometimes we see 72-year-olds who are more fit than 55-year-olds, and so it is best to look at functional capacity, and then to be as specific as possible when it comes to personalizing exercise for people with cancer. Otherwise we are sending mixed messages.?

Sprod will receive the ASCO Pain and Symptom Management Research Merit Award; she is one of four young investigators whose ASCO research presentations were selected for a high impact.

All of the exercise studies were funded through a variety of private and government sources, including the National Cancer Institute.

CSN: Bonds talking with Giants about joining staff

SAN FRANCISCO -- A significantly slimmer Barry Bonds made his first appearance of the 2012 season at AT&T Park Monday, and the Home Run King held court with a handful of reporters for about 35 minutes outside the Giants clubhouse, answering every question with direct eye contact and, sometimes, deep-throated laughs.

It was a 180-degree turn from his time as an active player, which included 15 seasons with the Giants and an often-rancorous relationship with the media.

Bonds, who said he was in attendance with a group of children in a charitable capacity, referred to himself as "a felon convicted of obstruction of justice" several times while acknowledging that his case is still in the appeal process.

"I'm a convicted felon of obstruction of justice and that's what I am," he said. "I live with it.

"It will never go off your mind. You never forget those things. You move on (but) I'll never forget it."

He was convicted in April of 2011.

"I was never convicted of steroids," he said

Bonds, who turns 48 on July 24, appeared to be in better shape than half of the Giants' current roster as he also spoke of his new obsession -- cycling, he estimated he biked 400 miles a week -- as helping him slim down, while saying he now weighs between 212-215 pounds.

"But I never weighed more than 238 when I was playing," he said, adding that he underwent surgery on his lower back and left hip in the past five weeks.

He said he spends his time these days watching "cycling, track and the Giants" on television.

"I'm following (Tim) Lincecum a lot," Bonds said. "I hope Brian Wilson comes back strong (from Tommy John Surgery), and I'm very proud of Matt Cain."

Bonds, who last played in the Major Leagues in 2007, when he batted .276? and hit 28 home runs with 66 RBI in 126 games to finish with the record 762 career homers, insisted he did not miss the game.

"No," he said, "I played a long time. I gave my life and my soul, my whole body on the baseball field, and I did it for 22 years?whether you hated me or liked me, you were there?I always played better on the edge."

Still, he figures he has something to give back and said he felt for Lincecum and his latest struggles.

"It happens," Bonds said. "It's how he deals with it. People start to figure things out. He's a wonderful pitcher and a great kid.

"I went through (struggles)."

Perhaps most intriguing, though, is Bonds' revelation that he has talked with Giants President and CEO Larry Baer about re-joining the club, perhaps as a roving instructor, of some sort, "helping major leaguers (and) minor leaguers" in the Giants organization.

"My expertise is baseball," he said.

"If you believe that I can contribute and help the organization, then fine. If you don't, fine. I'm just saying it's out there."

His appeal, both legally and to the Giants front office, notwithstanding, this winter figures to get interesting for Bonds as his name appears for the first time on the Hall of Fame ballot, as do several other key players from the game's Steroid Era such as Roger Clemens and Sammy Sosa.

Does the seven-time National League MVP expect to be a first-ballot Hall of Famer?

"I have no idea," he said. "That's up to the writers. I don't worry about that stuff. I'll be at home having a good time with my kids. Or hopefully here. Doing some stuff for other kids. That'll take care of itself. I don't need to worry about that stuff.

"You guys know me. I don't try to predict the future or analyze how other people are thinking or how it's going to turn out?it would be very sad if it didn't happen. That's why I don't need to comment on it. There's really no need."

He had already spoken volumes.

(AP) ? There was Brad Pitt and Nicole Kidman, red carpet glamour and a crop of new Academy Award contenders ? but this was also the year the global financial crisis exploded onto movie screens at Cannes.

"La Crise" ? as the French call it ? bedeviled Robert Pattinson's disaster-bound billionaire in David Cronenberg's "Cosmopolis," the unemployed Glasgow youth in Ken Loach's "The Angels' Share," the bare-knuckle boxer in Jacques Audiard's "Rust and Bone" and the worried mobsters in Andrew Dominik's "Killing Them Softly."

We live in anxious times, and that feeling was reflected at the French Riviera film festival that's a byword for frocks and froth, as well as for serious cinema.

The mood seemed to be mirrored by the weather. Several days were unseasonably cold and stormy, turning red-carpet photocalls into rain-lashed ordeals.

In the face of this angst, the jury rewarded love, giving Cannes' top prize, the Palme d'Or, to Austrian director Michael Haneke for "Amour," a starkly powerful film about an elderly couple coping with the wife's worsening health.

Second and third prizes went to Matteo Garrone's Italian satire "Reality" and Ken Loach's whiskey-tasting comedy "The Angels' Share," and there were acting honors for Denmark's Mads Mikkelsen for "The Hunt" and Romania's Cristina Flutur and Cosmina Stratan for "Beyond the Hills."

Although the festival had a strong American flavor, there were no prizes for a batch of films that examined the United States, past and present ? often through the lens of non-American directors.

Australia's John Hillcoat depicted Prohibition-era bootleggers in "Lawless" and Brazil's Walter Salles crossed the country in his adaptation of Jack Kerouac's Beat classic "On the Road." New Zealand-born Dominik set "Killing Them Softly," a thriller starring Brad Pitt as a worldly Mob enforcer, against the backdrop of the 2008 U.S. presidential election, while Canada's Cronenberg sent Pattinson's stretch limo across a Manhattan of security threats and Occupy-style protests.

Cronenberg said that "it seemed at points that we were working more on a documentary."

American directors also looked long and hard at their country. Lee Daniels stirred the sexual and racial politics of the 1960s South into a death-row thriller in "The Paperboy," while Jeff Nichols' "Mud" spun a modern-day "Huckleberry Finn" story among Mississippi River fishing families whose way of life is threatened.

It would not be the Cannes Film Festival without moments of controversy ? and craziness.

The former was provided by the absence of any female directors from the 22 films in the festival's main competition. The situation drew letters and petitions in France and the United States, and even a small protest by feminist group La Barbe in front of Cannes' famous red carpet.

Cannes director Thierry Fremaux responded that he chose films solely on merit, but the festival promised to make a greater effort to hunt down films by women.

The baffling and bizarre were provided by the surreal appearance of the devil in a Mexican family home in Carlos Reygadas' "Post Tenebras Lux" ? which won the directing prize ? and by pretty much everything, including a parking lot full of talking stretch limos, in Leos Carax's "Holy Motors."

While Carax's mysterious meditation on performance and reality is unlikely to tempt Hollywood's Academy, there was plenty at Cannes that will.

There was no film this year with the obvious mainstream crossover appeal of last year's breakout Cannes movie, "The Artist," which went on to win five Oscars.

That film was acquired by The Weinstein Co., which this year has picked up "The Sapphires," a buoyant musical about an Australian Aboriginal girl group that played out of competition at Cannes. It plans a fall release in the U.S.

The likeliest candidate for an Oscars boost may be "Mud," an assured and moving third feature from 33-year-old director Nichol. The film gives Matthew McConaughey a standout role as a mystic-minded fugitive holed up on an island in the Mississippi, and also draws powerfully natural performances from child actors Tye Sheridan and Jacob Lofland.

McConaughey also appears in "The Paperboy," which features an attention-grabbing performance from Kidman as a sultry femme fatale. It could win her an Oscar nomination, and there could also be one for Marion Cotillard's intense performance as a killer whale trainer who has a tragic workplace accident in "Rust and Bone."

And there's likely to be a solid audience for "Killing Them Softly," a taut, 1970s-style crime thriller that sees Pitt play cynical straight man to some outstanding character acting from the likes of James Gandolfini and Ray Liotta.

Cannes is a strange blend of high-art seriousness and Hollywood chutzpah, where the latest Haneke masterwork coexists with Sacha Baron Cohen riding a camel down the seaside Croisette as "The Dictator."

Even for showbiz veterans, it can be a remarkable experience, as McConaughey discovered at the gala premiere of "The Paperboy."

"It got a wonderful ovation and I've never experienced that," he said. "I've never done stage, and so I've never really experienced an immediate response live like that. It was nice to take a breath and say, 'Feel this. Feel this, McConaughey. This is a special, once-in-a-lifetime thing.'"

Brad Pitt mob movie portrays broken American dream

CANNES, France (Reuters) - Brad Pitt's latest movie paints a bleak picture of the broken American dream, blending a violent but comic gangster story with overt criticism of politicians' failure to address the economic crisis.

"Killing Them Softly" takes place in an unspecified U.S. city which has borne the brunt of the financial collapse -- houses are abandoned, shops are shuttered and petty criminals and mobsters alike are struggling to get by.

The movie, co-produced by Pitt, is in the main competition at the Cannes film festival this year, and has its red carpet world premiere on Tuesday.

Pitt plays ruthless hitman Jackie Cogan, brought in by a syndicate of mafia bosses to eliminate a group of thieves who raid a high-stakes poker game.

The title derives from his insistence on avoiding unnecessary pain and suffering when he carries out his killings.

It features gangster movie mainstays Ray Liotta and James Gandolfini, and reunites Pitt with New Zealand-born director Andrew Dominik after the two collaborated on "The Assassination of Jesse James by the Coward Robert Ford".

The political message of the film is unavoidable. News channels play in the background in bars and on the radio in cars, and the topic of debate is invariably the financial crisis, political failure, greed and shattered dreams.

Barack Obama, John McCain and George W. Bush appear on the 2008 campaign trail making promises to address the economy and preserve the ideals on which the country was built.

In a scene at the end, Cogan launches a scathing attack on Thomas Jefferson, the main author of the U.S. Declaration of Independence, whom he accuses of being a liar and hypocrite.

"I live in America and in America you're on your own," Pitt's character declares. "America's not a country, it's just a business."

At a news conference following a press screening of the film, Pitt said he did not want Killing Them Softly to be seen as an attack on President Obama.

"I lean more towards the left and I want to understand my own bias and so I am not opposed to characters that have different views from yourself," he said.

"I think very highly of him (Jefferson) actually.

"We are playing people with very specific opinions. We are clearly living in our country at a time of great divide and so I'm interested in those other arguments that are ... certainly not mine."

He spoke of a "toxic" political divide in the United States where "it's more about the party winning the argument than about the issues themselves. It's a serious, serious problem."

Pitt did not seem surprised when questions turned from the film and its political message to his personal life.

He told journalists that his fiancee Angelina Jolie was not in Cannes, quelling rumors that the Hollywood power couple would appear together on the red carpet.

Asked when they planned to get married, he replied: "We have no date. We actually really, truly have no date. Certainly date-wise it's absolutely rumor.

"And I'm still hoping we can figure out our marriage equality in the States before that date," he added, referring to his support for same-sex marriage countrywide.

Liotta, best known as a nasty mobster, said it made a welcome change to be on the receiving end of a cinematic beating. In Killing Them softly he plays the likeable Markie, who is framed and subsequently punished for the poker heist.

"What I really liked about it was I'm usually the one beating people up so it was nice to go the other way and have them beat me up, it was really a nice change.

"The hardest part was letting those two guys beat me up because I know I can take them. That's the roughest part, these little shrimps beating me up."

(AP) ? Forecasters late Saturday issued a tropical storm watch for parts of the South Carolina coast after the first tropical storm of the 2012 hurricane season formed in the Atlantic.

The National Hurricane Center in Miami issued the watch after Tropical Storm Alberto formed Saturday. The official start to hurricane season is June 1, but tropical storms occasionally occur before then.

The advisory covers the South Carolina coast from the Savannah River to the South Santee River for at least the next 24 hours. Tropical storm and dangerous surf conditions are possible.

Late Saturday, the storm was 110 miles (177 kilometers) southeast of Charleston, S.C. with maximum sustained winds of 50 mph (80 kph). It was moving southwest at 6 mph (10 kph).

Forecasters advised coastal interests from Georgia to North Carolina's Outer Banks to track the storm.

The hurricane center said a decrease in speed was expected through Sunday, with the storm turning toward the west-northwest ? and then moving north-northeast by Monday. Tropical storm-force winds extend outward up to 45 miles (72 kilometers).

National Weather Service meteorologist Sandy LaCorte in Wilmington said early Saturday evening the system is expected to reverse course and head northeast over the next several days. She said the center of the storm is not expected to get close to the Carolinas' coast.

LaCorte said Alberto will produce increased waves at beaches in the Carolinas. There is a high risk of rip currents along North Carolina's Outer Banks, and a moderate risk along the southeastern beaches and the entire South Carolina coast. Winds will gust to around 25 mph.

The weather service said there will be isolated and scattered rain showers along the coast of the Carolinas into early next week.

A forecast map by the hurricane center predicts that the storm will drift toward the open sea off the Mid-Atlantic region by midweek, but it's difficult to accurately predict a storm's path days in advance.

Major developers including Barratt, Taylor Wimpey and Berkeley have stepped up efforts to court cash-rich Far Eastern buyers since 2009 after the global financial crisis sapped demand at home. Developers do not all use exactly the same marketing methods.

"It (the development) has excellent transport links and easy access to well-known shops; the distances to which are clearly marked in our brochure. In addition, by far the majority of buyers have or will visit our developments before buying."

Ingrid Skinner, managing director of Taylor Wimpey Central London, said: "Buyers need to be able to trust the company they are buying from. At Taylor Wimpey we offer the same high level of service to all of our customers."
